Waar je werkt
De positie ontstaat binnen FACT Ouderen. Dynamischer dan dit vind je het niet binnen de ggz Het team FACT Ouderen is er voor cliënten met een ernstige psychische aandoening (EPA) die 60 jaar of ouder zijn. Je werkt in een multidisciplinair team, waarbij je dankzij het shared caseload-principe ook echt als een team werkt. Binnen het team werken nu een verpleegkundig specialist, psychiater, GZ-psycholoog, SPV, verpleegkundigen en Maatschappelijk werk. Door groei van het aantal cliënten en het streven om dit jaar meer samen te gaan werken met onze collega's van de poli Ouderen gaan we het team uitbreiden. Jouw nieuwe collega's lopen warm voor hun werk, zijn behulpzaam en oplossingsgericht. Ze werken samen om de meest optimale zorg te leveren en nemen het voortouw om deze continu te verbeteren. Wat je doet
De Sociaal Psychiatrisch Verpleegkundige (SPV)/casemanager coördineert, behandelt en begeleidt de cliënt en zijn/haar netwerk vanuit een multidisciplinair team en een ambulante setting. Hierbij maak je als SPV gebruik van verschillende therapeutische deeltechnieken zoals cognitieve gedragstherapie, systeemtherapie, motiverende gespreksvoering en probleemoplossende gesprekstechnieken gericht op crisisinterventie, stabilisering en terugvalpreventie. Je verricht je intakes en indicatiestellingen met een klinisch psycholoog of psychiater als regiebehandelaar en vervult eventueel ook de voorwacht crisisdienst binnen kantoortijden. De casemanager (coördinerend verpleegkundige) heeft zijn eigen caseload en treedt naar voren in de rol als behandel coördinator. Ook kan als co-therapeut worden deelgenomen aan groepen. Je werkt nauw samen met de psychiater/KP en de GZ-psycholoog.
